One row per store, with a header row. Name and address are enough; phone, email, website, category and coordinates are optional. Download the template from the Import tab and fill it in.
Yes. It uses the public OpenStreetMap service, one address per second, and only while you import. Visitors never trigger it. With thousands of rows, put the coordinates in the file and it is instant.
